My son/daughter participating in the Farsi classes to learn Farsi, Islamic religious education, and cultural activities run by Preston Hazara Community Association. I understand that every care will be taken to ensure the health, safety, and welfare of my child. I realise and accept that if my child’s behaviour adversely affects the safety of others or the activities, the organisers reserve the right to send my child home. I also confirm that I will be present at the centre to supervise my child/children during the Farsi classes and any other activities taking place at the centre. I assume full responsibility for my child/children for any accidents that may occur.
